MINOR PARTICIPANT WAIVER, RELEASE & MEDIA AUTHORIZATION**

Ongoing Authorization

This Waiver and Release is valid for all current and future PropurForm LLC training sessions, programs, camps, clinics, evaluations, and events, unless revoked in writing.

ASSUMPTION OF RISK

I understand that participation in basketball training and athletic activities involves inherent risks, including but not limited to physical injury, illness, or other medical conditions. I voluntarily allow my child to participate and assume all risks associated with participation in PropurForm LLC activities.

RELEASE OF LIABILITY

I, on behalf of myself and my minor child, hereby release, waive, discharge, and hold harmless PropurForm LLC, its owners, officers, employees, contractors, volunteers, agents, and facility partners from any and all claims, liabilities, demands, or causes of action arising from participation in PropurForm activities, including but not limited to injury, illness, or property damage, whether caused by negligence or otherwise, to the fullest extent permitted by law.

MEDICAL AUTHORIZATION

In the event of an emergency and if I cannot be reached, I authorize PropurForm LLC and its representatives to secure emergency medical treatment for my child. I understand that I am financially responsible for any medical services provided.

MEDIA RELEASE

I grant permission to PropurForm LLC to photograph, record, and use my child’s name, image, likeness, voice, and video in any media format for promotional, educational, marketing, or archival purposes, without compensation.

CODE OF CONDUCT & REMOVAL

I understand that PropurForm LLC reserves the right to remove any participant whose behavior is deemed unsafe, disruptive, or contrary to the values of Professionalism, Purpose, and Performance.

GOVERNING LAW

This agreement shall be governed by and interpreted under the laws of the Commonwealth of Massachusetts.

ACKNOWLEDGMENT & SIGNATURE

I certify that I am the legal parent or guardian of the above-named minor and that I have read, understand, and agree to all terms of this waiver.
